Emergency Consent and Information Form
Required for ALL students — Until this form is received, no walks or excursions off campus, even on the first day of school, will be permitted. Middle School student forms must be on file in the office before participating in Project Week activities. DUE JUNE 1 – Please note this change from past years.
Authorization to Administer Medicine
If your child is to receive any medication during school hours (Tylenol, Aspirin, cough medicine, etc.), this form must be completed and signed by both parent and doctor. Due JULY 1.
Health Record/Medical Assessment
Required for returning students entering Grades 3 & 7 and ALL new students. Parent fills out Part I; the physician fills out Part II. DUE JULY 1.
Student Diversity Information Card
for all new students – data required by the National Association of Independent Schools.
